Our Medical Social Work services provide counseling, long-term planning, education and referrals for benefits and entitlements, community resource linkage, placement and housing referral advocacy, discharge planning and brief care-focused family counseling.

- Information and referrals for clients and their families

- Those with significant challenges and life stressors such as limited or distant caregiving supports
- Clients struggling with adjustment to physical, social economic and emotional circumstances
- Clients in need of long-term in home care plans
- Consumers with a need and desire to be linked to resources and benefits

- Satisfactory progress in the home care plan
- A safe discharge with appropriate support services in place to meet long-term needs and reduce risk of future decline
Our Medical Social Workers are Masters Degree prepared in Social Work with health care experience. Additionally, they are required to participate in multiple yearly inservices and continuing education programs. They actively participate in professional organizations and advisory councils. Berks Visiting Nurse Association is a field supervisory site for local universities. Supervision is performed by a licensed MSW with many years of experience and ACSW accreditation.
